The Vital Statistics office will be closed each day from 11:30 - 12:30 for lunch.  Normal hours are Mon-Fri, 8 AM - 4 PM, closing for lunch from 11:30 until 12:30. Same day service ends at 3:45 p.m.

Certified copy fees for a vital record are $30, effective October 16, 2009.  The increase is a result of the $5 certified copy fee increase approved with the passage of the 2010 State budget.

The Fairfield Department of Health's Vital Statistics division issues certified copies of certificates for births, deaths, and fetal deaths occurring from 1908 to present in Fairfield County only.  Our registrar can assist you with:

• Registration of birth and death records

• Issuance of burial transit permits

• Completion of "Acknowledgement of Paternity Affidavits"

• Corrections to birth and death records

To obtain a Birth or Death Certificate:

Complete an application for a birth or death certificate.  Applications can be mailed, faxed, or completed in person at the Fairfield Department of Health. 

To obtain a copy of a certified birth or death certificate by mail, you must send a letter of request with a self addressed stamped envelope, and a check made payable to the Fairfield Department of Health for $30.00. Certificates requested via US Mail have a next day turn around.  Birth certificates for newborn babies will be ready 2 weeks after the birth.

The Fairfield Department of Health will accept personal checks if the person's address and phone number are on the check. Cash, Money Orders, Visa and Mastercard are also accepted.
