FAIRFIELD DEPARTMENT OF HEALTH: RECORDS REQUEST
Disclaimer: A person requesting public records need not identify themselves or the reason for which they seek the record. Providing such information to the records clerk is entirely voluntary, although it can often clarify and expedite the release of information. Public records requests can be made in person, over the telephone or online. Records will be released in accordance with the provisions of the Ohio Public Records Act, and may be subject to legal review prior to release.
Public Records copies are provided at $0.10 a page. Other mediums such as DVD, USB drive, if available may be a different charge. The records clerk may ask for prepayment before a request is processed. Payment may be made over the phone or by mail. The forms of payments are: Cash, Check, Money order, Visa or MasterCard, (checks and Money orders made payable to Fairfield Department of Health and sent to 1550 Sheridan Dr., Suite 100 Lancaster Ohio 43130) or in person at the health department Administration office with the Records Management Clerk.
