The Fairfield County Community Health Center
provides primary health care services for Fairfield County
residents. The Health Center is open Monday through Friday from 8
am to 4 pm by appointment. There are also evening hours on
the first and third Tuesday of the month. The Health
Center sees
patients with any chronic illness including diabetes and
hypertension, as well as immediate health concerns like
sinusitis and intestinal flu. The Health Center accepts
Medicaid, Medicare, CareSource and
Molina. The Health Center also accepts self-pay on a sliding fee
scale which is based on proof of income and family size.
The Fairfield
Department of Health (FDH) is pleased announce that it has been
granted Federally Qualified
Health Center “look alike” (FQHC-LA) status by the Centers
for Medicare and Medicaid Services (CMS).
Terresa L. Hawthorne, M.D., M.P.H. is the primary care practice
physician for the Health Center. Dr. Hawthorne
comes to the Fairfield Department of Health with a background as
a public health physician in Ross County’s Primary Care Clinic.
She is a graduate of The Ohio State University College of
Medicine, and has an added Masters in Public Health in
Occupational Medicine from the Medical College of Wisconsin.
She is Board Certified in General Practice, and is Board
Eligible in Preventive and Occupational Medicine. “This is
a wonderful opportunity for me to work in an environment where I
can be an advocate for the patient. The staff here go out
of their way for our patients, and treat everyone with care,”
says Dr. Hawthorne.
