The Fairfield Community Health Center provides primary health care
services for
Fairfield
County
residents. The Health Center is open Monday, Wednesday,
and Thursday from
8 am to 4 pm, Tuesday from 8 am to 5 pm, and Friday from 8 am to
3 pm. The Health Center is closed everyday from 11:45 - 12:45. There are also evening hours on
the first and third Tuesday of the month. The Health Center
sees patients with any chronic illness including diabetes and
hypertension, as well as immediate health concerns like
sinusitis and intestinal flu. The Health Center accepts
Medicaid, Medicare, CareSource and Molina. The
Health
Center
also accepts self-pay on a sliding fee scale which is based on
proof of income and family size.
The Fairfield
Department of Health (FDH) is pleased to announce that it has
been granted full Federally
Qualified Health Center (FQHC) status by the Department of Health and Human
Services, Health Resources and Services Administration.
(HRSA).
Terresa
L. Hawthorne, M.D., M.P.H. is the primary care practice
physician for the Health Center. Dr. Hawthorne comes to the
Fairfield Department of Health with a background as a public
health
physician at the Ross County Primary Care Clinic. She
is a graduate of The Ohio State University College of Medicine,
and has an added Masters Degree in Public Health and
Occupational Medicine from the Medical College of Wisconsin.
She is Board Certified in General Practice, and is Board
Eligible in Preventive and Occupational Medicine. “This is a
wonderful opportunity for me to work in an environment where I
can be an advocate for the patient. The staff here go out of
their way for our patients, and treat everyone with care,” says
Dr. Hawthorne.
Bernadette
Anderson, M.D., M.P.H, pictured at right, is Board Certified in
Family Practice, and works on Fridays in the primary
care clinic and in women’s health services.
