Please enter the township name and address where the nuisance is located.  If you do not know the address, give directions to the nuisance site.  Nearby addresses and cross streets are also helpful. 

The complainant is the person filling out the form.  

Lack of information on the form can delay the processing of the complaint please be as thorough as possible.   

Once the Nuisance complaint form is received by the Fairfield Department of Health a Sanitarian will be assigned to investigate the complaint.   The sanitarian will communicate primarily with the Offender to resolve the complaint.  If you have questions regarding a nuisance complaint you have submitted please call or email the department of health for an update.
